This paper constitutes an unusual opportunity to venture where few care to tread. It therefore calls for special attention to the structure of the presentation. This is organized in three major parts. This first part might be thought of as a meta-reflection about the challenges of presenting the theme. The second is the core content, whose structure is reviewed in the first. The third part contains the many detailed notes to underpin the arguments and suggest future lines of enquiry.
